All you have to do is scroll to the bottom of this page and click on the link to register & to become a member with Jack FM!
Make sure you put all info in correctly, including phone number so we can call you if & when we select your name as our winner!
Our winner will be selected at random on November 20th, 2018 @ 8 AM and contacted via phone.
Winner will have 24 hours to respond to claim their tickets. If there is no response within that time frame tickets will be forfeited back to station.
Our winner MUST pick up the tickets at our station: 450 Leonard Avenue, Fairmont WV during business hours:
Monday-Friday (8AM – 4:30 PM). We will NOT mail the tickets.
Winner must also present Valid Photo ID upon arrival to receive their tickets.
